Interested candidate shall fill the attached forms (complete in all aspects) and submit in the admission office of the Institute before closing date with attested photo copies of the following documents and recent passport size photographs.

1. Qualification certificate
2. CNIC/Form B
3. Domicile certificate
4. Passport size photographs
5. Affidavit on stamp paper of Rs.50/-
